Coach Forms Required by Gotham City Thunder and Red Lion Youth Lacrosse

Act 151 Form, Pa Child Abuse History Clearance
Act 34 Form, Pa Criminal Record Check
These forms need to be completed by all coaches, Asst Coaches and parents on the players sideline. If you have a current copy with in 6 month of the season starting you just need to supply a copy of your current forms. You are responsible for these forms and fees.
